About Care Coordination Department at NAMI Chicago

When the mental health system feels overwhelming, you don’t have to navigate it alone. Our Care Coordination team works with individuals living with serious mental illness and the families who support them, especially when they have experienced repeated hospitalizations, law enforcement involvement, or ongoing crisis.

Care Coordination services

We work alongside you to:

Stabilize

We address immediate concerns such as safety planning, hospital coordination, and urgent system navigation.

Plan

Once the immediate crisis has stabilized, we work with you to set realistic, achievable goals.

Engage

We provide ongoing advocacy and resource navigation to help you move toward greater stability.

Maintain

We offer extended support as needed for individuals and families facing prolonged or complex crises.

Suzanne’s Story

For nearly 17 years, Suzanne supported her daughter, who lives with schizophrenia. There were multiple hospitalizations and repeated crises that left Suzanne feeling exhausted and unsure where to turn. In March 2022, she contacted NAMI Chicago.

Is this program voluntary?
Yes. Individuals and families choose to work with us and may end services at any time.
Care Coordination is designed for individuals living with serious mental illness who have experienced repeated hospitalizations, law enforcement involvement related to mental health crises, or ongoing instability in outpatient settings.
No. We provide education and advocacy, but we do not control court decisions or mandate care.
No. This support is provided at no cost.
